How to Prescribe a Compounded Medication
(for Physicians and Practioners)
* Please note that Compound Pharmaceutical Technologies (CPT) is not an internet
pharmacy. You cannot order or prescribe any medication via this website.
CPT only engages in compounding in response to a doctor's prescription. Please
contact your physician to obtain a prescription. You may come to our location
during normal business hours to fill your prescription.
Option 1:
▫ Use the Compound Prescription Request form and fax it to Compound
Pharmaceutical Technologies at 866-684-MEDS (6337). Please fax a patient
demographic sheet to ensure timely service.
▫ Explain to patient that they have been prescribed a compounded prescription which is
being made especially for their condition and to be expecting a call from Compound
Pharmaceutical Technologies.
▫ Compound Pharmaceutical Technologies staff contacts patient, verifies address,
processes insurance then ships (at no extra charge) to the patient's home.
Option 2:
▫ Write the prescription on your normal prescription pad and fax it to Compound
Pharmaceutical Technologies at 866-684-MEDS (6337). Please fax a patient
demographic sheet to ensure timely service.
▫ Explain to patient that they have been prescribed a compounded prescription which is
being made especially for their condition and to be expecting a call from Compound
Pharmaceutical Technologies.
▫ Compound Pharmaceutical Technologies staff contacts patient, verifies address,
processes insurance then ships (at no extra charge) to the patient's home.
Option 3:
▫ Patient may bring the prescription to our pharmacy and pick up on the same day in
most circumstances.
We accept Workers Compensation and most Major Insurances. Medicare and
Medicaid do not pay for compounded prescriptions. We offer a 30% discount to
Senior Citizen and all cash customers.
